I just turned 18 this year. I have hidden double eyelids, and my eyes look quite sleepy, so I am considering eyelid surgery to make them look more alert. However, I have heard that having eyelid surgery too early may not be good. So doctor, may I ask at what age eyelid surgery can be performed, and if doing it at a young age will affect eye development or not?

Hello, many young people who notice uneven eyelids, small eyes, or early eyelid sagging often ask doctors questions such as “At what age can eyelid surgery be done?” or “Should people under 18 undergo eyelid surgery?” This is a very valid question, because not everyone is suitable for this procedure. Although eyelid surgery is a minor operation, it still requires careful consideration regarding age, overall health, and eye structure development. The doctor will explain this in detail for you below.

At what age can eyelid surgery be performed?

From a medical perspective, the most appropriate age for eyelid surgery is 18 years and older—when the body has fully developed, the eyelid structure is stable, and there are no significant ongoing changes. At this age, surgery can achieve optimal results, creating clear, balanced eyelid folds that are long-lasting without affecting the development of muscles, skin, or nerves around the eyes.

However, turning 18 does not automatically mean that eyelid surgery can be performed immediately. The doctor must also evaluate the actual eyelid condition:

  • If the patient has congenital single eyelids, uneven eyelids, or mild ptosis, the procedure can be performed safely.
  • If the eyelids are just slightly small and not yet stable due to ongoing natural development, it is advisable to wait longer to avoid future changes.

For adults, especially those over 25, eyelid surgery is often indicated to remove excess skin and fat in the eyelid area, helping to rejuvenate the eyes. For younger individuals, the procedure mainly focuses on aesthetic reshaping to create clearer, larger, and brighter-looking eyes.

Therefore, the most accurate answer to the question “At what age can eyelid surgery be performed?” is from 18 years old and above, after a specialist has carefully evaluated eye structure, skin condition, and overall health.

Can people under 18 undergo eyelid surgery?

In principle, individuals under 18 should not undergo eyelid surgery, except in special cases with clear medical indications. This is because, at this stage, the orbital bone structure and periocular tissues are still developing, and surgical intervention may cause changes to the eyelid fold over time, leading to asymmetry or the need for revision later.

However, there are certain exceptions, such as:

  • Children with severe congenital ptosis that covers the pupil and affects vision.
  • Congenital eyelid deformities that obstruct vision, in which case ophthalmology–aesthetic specialists may indicate corrective eyelid surgery for therapeutic rather than cosmetic purposes.

In these cases, surgery must be performed at a specialized hospital with an experienced team of oculoplastic surgeons and anesthesia specialists to ensure maximum safety for the patient.

Potential risks of undergoing eyelid surgery at too young an age

When eyelid surgery is performed at an excessively young age, especially under 18, several risks may occur:

  • Changes in the eyelid fold over time: As eyelid muscles and skin continue to develop, the created fold may shift, become uneven, or fade.
  • Impact on levator muscle function: If surgical techniques are not precise, it may cause weakness of the levator muscle, leading to ptosis or incomplete eye closure.
  • Unfavorable scarring or eyelid deformity: Adolescent skin is more prone to hypertrophic scarring or contracture if postoperative care is inadequate.
  • Psychological impact: At this age, young people are often influenced by social media and may choose surgery impulsively, even when it is not truly necessary.

For these reasons, doctors always advise against undergoing eyelid surgery too early, recommending that patients wait until eye development stabilizes and choose reputable facilities where they can receive direct consultation from specialists.

In summary, 18 years old and above is considered the safe age for eyelid surgery, as the eyelid structure has fully developed. For individuals under 18, intervention should only be considered when there are clear medical indications and must be prescribed by a specialized aesthetic eye surgeon. At Gangwhoo Cosmetic Hospital, doctors strictly adhere to preoperative screening protocols and apply exclusive Plasma microsurgical endoscopic eyelid surgery technology, ensuring gentle procedures, minimal pain, no visible scarring, and exceptional safety for all appropriately indicated age groups.
