I currently have tooth decay in a few molars, but I also want to get braces as soon as possible. I would like to know whether it is possible to get braces if you have tooth decay, or if it is mandatory to completely treat the decay before starting orthodontic treatment. During the orthodontic process, what should I pay attention to in order to prevent the decay from worsening?

Hello, many people considering orthodontic treatment are concerned about whether tooth decay will affect their ability to get braces. This is a completely valid concern because orthodontic treatment is a long-term process that requires a healthy oral foundation. The doctor will explain each aspect in detail so that you clearly understand whether you can get braces if you have tooth decay, when orthodontic treatment can proceed, and when dental treatment is required first to ensure safety.

Can You Get Braces If You Have Tooth Decay?

The answer is as follows: it is still possible to get braces if you have tooth decay, but not every case is suitable for immediate orthodontic treatment.

According to orthodontic principles, the teeth and surrounding tissues (enamel, dentin, dental pulp, gums, and alveolar bone) must be healthy before orthodontic appliances are placed. Therefore, regarding the questions whether you can get braces if you have tooth decay or whether a decayed tooth can be treated with braces, the doctor’s direct answer is:

  • If the decay is mild and has not affected the dental pulp, it can be treated quickly before proceeding with orthodontic treatment.
  • If the decayed tooth has not been treated, the doctor will not immediately place braces or aligners.

In other words, having braces while experiencing tooth decay is not an absolute contraindication, but tooth decay must be treated before orthodontic treatment begins.

Why Must Tooth Decay Be Treated Before Getting Braces?

According to medical standards, treating tooth decay before orthodontic treatment is mandatory to prevent complications throughout the orthodontic process, which may last from 1 to 3 years.

To help you understand, when braces are applied, continuous force is exerted on the teeth. If a tooth is already decayed, the risk of pain and pulp inflammation increases significantly. Brackets or aligners can also make oral hygiene more challenging, allowing cavities to progress more rapidly. If tooth decay is left untreated, it may lead to pulpitis, dental abscesses, and even require the removal of orthodontic appliances during treatment.

For this reason, to ensure safe and stable orthodontic results, dentists always prioritize completely treating tooth decay before starting orthodontic treatment.

The Process of Managing Tooth Decay Before Orthodontic Treatment

For patients wondering whether a decayed tooth can be treated with braces, orthodontists typically follow the standard process below:

  • Comprehensive oral examination to assess the severity of tooth decay
  • Dental X-rays to evaluate dentin and pulp involvement
  • Dental fillings for mild cavities
  • Root canal treatment and restoration if the decay has reached the pulp
  • Monitoring the tooth after treatment to ensure stability before placing orthodontic appliances

This process ensures that the teeth are healthy enough to withstand long-term orthodontic forces without complications.

Can Severe Tooth Decay Still Be Treated with Braces?

This is a concern for many patients. In cases of severe tooth decay, the orthodontist will carefully evaluate the condition before deciding whether orthodontic treatment is appropriate.

Specifically:

  • If the severely decayed tooth can still be preserved, the doctor will perform root canal treatment, place a dental crown, or restore the tooth before orthodontic treatment.
  • If the tooth is extensively damaged with a poor prognosis, extraction may be necessary and incorporated into an appropriate orthodontic treatment plan.

In these situations, orthodontic treatment can still be performed, but the treatment plan will be personalized to ensure both optimal aesthetics and proper chewing function after completion.

Oral Care Tips for Patients with Both Tooth Decay and Braces

For patients who are currently undergoing orthodontic treatment with a history of tooth decay or who are prone to cavities, the doctor recommends paying special attention to the following:

  • Brush your teeth properly at least 2–3 times per day
  • Use interdental brushes and water flossers to clean around brackets and wires
  • Limit sugary foods, carbonated beverages, and sticky carbohydrates
  • Attend regular follow-up appointments to monitor for recurrent decay
  • Notify your orthodontist immediately if you experience unusual tooth pain or sensitivity

Proper oral care helps minimize the risk of tooth decay progressing throughout the orthodontic treatment process.
